Student Enrollment Agreement    

Thank you for choosing Okami Martial Arts - Gracie Jiu Jitsu Saskatoon ! We pride ourselves in providing the safest and most structured learning opportunity in all of Saskatoon for Muay Thai, Jiu Jitsu, Karate and Sambo.  We are very excited to have you join the family. Please review and acknowledge the following so we can get you started on the Okami Martial Arts journey. 

I, THE STUDENT OR THE PARENT ON BEHALF OF THE STUDENT, UNDERSTAND that under the terms of this agreement, the school obligates itself to furnish me/my child with competent instruction and suitable facilities for teaching lessons. All class lessons are supervised by qualified personnel trained in the procedures and traditions of Okami Martial Arts.  I agree that I/my child is physically fit to receive and participate in the prescribed course of instruction. I agree to faithfully comply with all the rules and regulations of Okami Martial Arts - Gracie Jiu Jitsu Saskatoon . The regular hourly tuition is $40 per hour; however, a substantial discount has been provided to me by enrolling in a training program.

LIABILITY, WAIVER AND RELEASE : I understand and acknowledge that strict observation of the Okami Martial Arts - Gracie Jiu Jitsu Saskatoon rules and regulations, including the rules and regulations relative to training and the use of protective equipment, is required. I understand and agree that the use of Okami Martial Arts - Gracie Jiu Jitsu Saskatoon facilities and my/my child's presence at the facility are at the sole risk of myself.  I also understand that the martial arts involves skills and training which include violent and sudden movements, and that in connection with the training and instruction, there will be physical contact between instructors and members and myself/my child.   I understand that such contact may result in personal injury to myself/my child, despite precautions taken by Okami Martial Arts - Gracie Jiu Jitsu Saskatoon to avoid such injuries. Myself/my child  acknowledges martial arts instruction has the potential for serious injuries including but not limited to knee, head, neck, joint or spinal injuries, and includes cardiovascular exercise which should not be undertaken by individuals with heart defects or high blood pressure. individually on behalf of myself/my child and  hereby agrees to hold harmless, release, indemnify and forever discharge Okami Martial Arts - Gracie Jiu Jitsu Saskatoon its officers, directors and employees, instructors, agents and authorized representatives from any liability, claim or loss, including loss of property, damage, personal injury, or expense incurred by myself/my child and arising from mine/my childs execution of this Contract, participation in any program offered by Okami Martial Arts - Gracie Jiu Jitsu Saskatoon , including but not limited to, any injury or damage caused by the negligence or gross negligence of Okami Martial Arts - Gracie Jiu Jitsu Saskatoon and its instructors, agents, employees, operators, or authorized representatives. I specifically understand and agree that I am assuming the risk of any and all injuries that I/my child may suffer or incur as a result of mine/my childs participation of any program offered by the Okami Martial Arts - Gracie Jiu Jitsu Saskatoon therefore I agree and waive all rights for any legal actions or harm to the buyer. 

BY ENTERING INTO THE AGREEMENT I grant the school permission to use photographs, videotapes, artwork or other likenesses of me/my child (the student) for marketing, trade, publishing, or any other lawful purpose, without compensation.

TRIAL MEMBERSHIP IS non-transferable except as stated herein.

Membership Details  

I understand that my 10-day free trial period begins on {start_date} and will continue for a duration of 10 calendar days. I will NOT be charged for any classes during this trial period.  We recommend during your trial, you attend any and all classes that suit your age/interest to get the full understanding of what we offer.  I understand that my trial will not be extended for any reason.  

I have read and understand the terms and conditions of the 10-day free trial set forth herein.
