Student TRIAL Enrollment Agreement    

Thank you for choosing Okami Martial Arts! We are excited to welcome you to our family and provide a safe, structured introduction to Martial Arts.

Agreement & Waiver

• I confirm that I/my child am physically able to participate in martial arts training and will follow the rules and safety guidelines of Okami Martial Arts.

• I understand martial arts involves physical contact and carries a risk of injury. By signing, I assume all risks and release Okami Martial Arts, its owners, instructors, and staff from any liability for injury, loss, or damages.

• I authorize basic first aid or emergency care if needed and accept responsibility for related costs.

• I give permission for photos or videos of myself/my child taken in class to be used for school promotions.

• Trial memberships are non-transferable.

10-Day Free Trial

• I understand my 10-day free trial begins on {start_date} and lasts 10 calendar days. No charges will occur during this period.

• I understand that trial classes cannot be extended for any reason.

• I am encouraged to attend all classes suitable for my age and interest to get a full understanding of what Okami Martial Arts offers.
